New Approach to Kidney Care Available to Minnesotans on Allina Health | Aetna Plans with Evergreen Nephrology
The collaboration provides access to Evergreen’s unique kidney care model for Minnesotans with chronic kidney disease or end-stage renal disease
St. Louis Park, Minn. — Jan. 2, 2024 — Allina Health | Aetna, a Minnesota-based health insurance plan, is teaming up with Evergreen Nephrology, an organization specializing in kidney care, to provide innovative care for Allina Health | Aetna Medicare members with chronic and end-stage kidney disease.
Through this collaboration with Evergreen Nephrology, Allina Health | Aetna members will have access to a multi-dimensional approach to kidney care that goes beyond physical health to address holistic needs.

Evergreen works closely with nephrologists to provide personalized care plans to help improve the outcomes and quality of life for patients by:
- Delaying disease progression
- Shifting care to the home
- Addressing social determinants of health
